Case Manager - LTC and DD Waivers

Full-Time Case Manager needed to assess, create/review care plans and coordinate services with individuals who qualify for CADI, DD and/or BI waivers. Candidate will make referrals for individuals served to community resources based on their assessed need and ensure waiver coordination/documents meet necessary deadlines and meet lead agency guidelines.

HOURS AVAILABLE: Monday - Friday 8:00am - 4:30pm, with some flexibility. Starting wage $45,000+ based on education and experience.


WORK LOCATION: Primary location is North St. Paul, MN. Travel throughout the Twin Cities/Metro Area is required for meetings and face to face visits with individuals in the community. Based on the caseload, there may be travel outside the Metro Area.

JOB SUMMARY: FT Case Manager needed to assess, create/review care plans and coordinate services with individuals who qualify for CADI, DD and/or BI waivers. Candidate will make referrals for individuals served to community resources based on their assessed need and ensure waiver coordination/documents meet necessary deadlines and meet lead agency guidelines.

REQUIREMENTS:

  • 4 year degree in Human Services Field (Social Work, Psychology, Sociology) or closely related field.
  • 1 year experience working with individuals diagnosed with Mental Illness and/or Developmental Disability.
  • Driver's license, reliable vehicle and proof of insurance.
  • Reliably commute or relocate prior to hire in order to work with individuals in the Twin Cities/Metro Area.
  • Must clear background check.
  • Candidate should have good verbal and written communication, be organized, flexible and motivated.
  • Strong time management and multitasking skills are essential to this position.
  • Experience working with CADI, BI and/or DD waivers preferred.
