Case Manager - LTC and DD Waivers
Full-Time Case Manager needed to assess, create/review care plans and coordinate services with individuals who qualify for CADI, DD and/or BI waivers. Candidate will make referrals for individuals served to community resources based on their assessed need and ensure waiver coordination/documents meet necessary deadlines and meet lead agency guidelines.
HOURS AVAILABLE: Monday - Friday 8:00am - 4:30pm, with some flexibility. Starting wage $45,000+ based on education and experience. WORK LOCATION: Primary location is North St. Paul, MN. Travel throughout the Twin Cities/Metro Area is required for meetings and face to face visits with individuals in the community. Based on the caseload, there may be travel outside the Metro Area.
